Nếu gặp khó khăn trong quá trình thay đổi nơi đăng ký khám chữa bệnh BHYT, vui lòng liên hệ:
Thủ tục cần xuất trình: Gồm Thẻ bảo hiểm y tế ( còn thời hạn), Giấy tờ tùy thân ( dán ảnh hợp lệ do cơ quan có thẩm quyền cấp) . Ví dụ: CMND, hộ chiếu, bằng lái xe, thẻ đảng viên, thẻ đoàn viên công đoàn, thẻ hưu trí, thẻ học sinh, sinh viên… và hồ sơ chuyển viện theo quy định của Bộ y tế. Ngoài ra cũng cần giấy hẹn khám lại (trường hợp đến khám lại theo yêu cầu của Bác sỹ. Mỗi lần chuyển viện chỉ được hẹn khám lại theo chế độ BHYT một lần).
* Quyền lợi được hưởng
– 100% chi phí khám chữa bệnh BHYT đối với: Bà mẹ Việt Nam anh hùng, lão thành cách mạng, thương bệnh binh>81%, những người có công khác, lực lượng Công an nhân dân, Trẻ em dưới 6 tuổi với điều kiện Chi Phí cho một lần KCB thấp hơn 15% mức lương tối thiểu.
– 95% chi phí khám chữa bệnh BHYT đối với: Hưu trí, trợ cấp mất sức,hộ nghèo, dân tộc thiểu số
– 80% chi phí khám chữa bệnh BHYT đối với các đối tượng chuyển viện đúng tuyến.
* Thủ tục cần xuất trình : Gồm có thẻ Bảo hiểm y tế và Giấy tờ tùy thân (có dán ảnh)
* Quyền lợi được hưởng : Bệnh nhân được hưởng theo đúng mức hưởng 100%, 95%, 80% theo từng đối tượng (như phần 2a).
